• place your dimension on a new layer
• set the length of the extension lines not the gap
• drag the dimension text to the desired position
• override the dimension text if needed

and from my own experience

• moving and aligning dimension lines is easiest by scaling
• double click the next reference point to place continuous auto aligned dimension
